Florida Special Needs Registry
The Florida Department of Health, in coordination with county health departments and each local emergency management agency in the state, developed a registry for persons with special needs to register with their local emergency management agency to receive assistance during a disaster. The statewide registry provides first responders with valuable information to prepare for disasters or other emergencies.
Registration can be done online through https://snr.flhealthresponse.com/
A paper form is available on our website www.hardeecountyfl.gov/emergency-management or by visiting Emergency Management Office at 404 W. Orange Street Wauchula, FL 33873, or Department of Health at 115 K.D. Revell Road Wauchula, FL 33873. Completed forms will need to be returned to Emergency Management or Department of Health.
Completing the registration does not automatically qualify the individual for a special needs shelter.
Please first contact your health care provider or the local Department of Health if you are at risk for coronavirus (COVID-19 ) . Risk factors that may indicate COVID-19 include fever, cough, or flu-like signs, AND either recent international travel OR contact with someone recently diagnosed with COVID-19. Health care officials will direct you to the appropriate place for care. To help prevent the spread of COVID-19, you should avoid the emergency department at your local hospital except in the event of an emergency.
